Title: create input-form when operator is offline

Hi,
 I want to add an input in the chat window that appears when the operator is offline
Now i have:
mail, name and message, but i want to add phone in this window.

This is complicated if you dont know what you're doing... to give you a hint, you need to go into the style you're using, and then into templates, then in the mail.tpl.
